Dr. McHugh has served a variety of medical positions in his career. McHugh was the founder and first director of the Westchester Division of the Bourne Behavioral Research Laboratory of New York Hospital at Cornell and chairman of the Medical Board at Johns Hopkins Hospital. He managed the Blades Center for Clinical Practice and Research in Alcohol/Drug Dependence.

Currently Dr. McHugh is the director of the Department of Psychiatry and Behavioral Sciences at the Johns Hopkins University School of Medicine and Johns Hopkins Hospital and a professor at Johns Hopkins University School of Hygiene and Public Health.
